چکیده

Introduction During assisted mechanical ventilation, appropriate WOB prevents diaphragm atrophy and fastens weaning. During PSV the ventilator support is fixed, irrespective of the patient’s effort. On the contrary, both the NAVA and PAV + algorithms are designed to amplify patient’s spontaneous effort. [1], [2] Objectives To test if the assist ventilation mode algorithm has an impact on patient’s WOB.

Neurally adjusted ventilatory assist and proportional assist ventilation both improve patient-ventilator interaction

INTRODUCTION The objective was to compare the impact of three assistance levels of different modes of mechanical ventilation; neurally adjusted ventilatory assist (NAVA), proportional assist ventilation (PAV), and pressure support ventilation (PSV) on major features of patient-ventilator interaction. METHODS PSV, NAVA, and PAV were set to obtain a tidal volume (VT) of 6 to 8 ml/kg (PSV₁₀₀, NA...

Assisted mechanical ventilation: the future is now!

Assisted ventilation is a highly complex process that requires an intimate interaction between the ventilator and the patient. The complexity of this form of ventilation is frequently underappreciated by the bedside clinician. In assisted mechanical ventilation, regardless of the specific mode, the ventilator's gas delivery pattern and the patient's breathing pattern must match near perfectly o...

Impact of prolonged assisted ventilation on diaphragmatic efficiency: NAVA versus PSV

BACKGROUND Prolonged controlled mechanical ventilation depresses diaphragmatic efficiency. Assisted modes of ventilation should improve it. We assessed the impact of pressure support ventilation versus neurally adjusted ventilator assist on diaphragmatic efficiency. METHOD Patients previously ventilated with controlled mechanical ventilation for 72 hours or more were randomized to be ventilat...

Neurally adjusted ventilatory assist and pressure support ventilation in small species and the impact of instrumental dead space.

BACKGROUND Neurally adjusted ventilatory assist (NAVA) is a pneumatically-independent mode of mechanical ventilation controlled by diaphragm electrical activity (EAdi), and has not yet been implemented in very small species. OBJECTIVES The aims of the study were to evaluate the feasibility of applying NAVA in very small species and to compare this to pressure support ventilation (PSV) in term...

Can proportional ventilation modes facilitate exercise in critically ill patients? A physiological cross-over study

BACKGROUND Early exercise of critically ill patients may have beneficial effects on muscle strength, mass and systemic inflammation. During pressure support ventilation (PSV), a mismatch between demand and assist could increase work of breathing and limit exercise. A better exercise tolerance is possible with a proportional mode of ventilation (Proportional Assist Ventilation, PAV+ and Neurally...
